MovieExtras.ie Under 16 Members (Minors)

Due to Child Protection Legislation, there are a number of forms which need to be completed for a Minor who is under 16 years of age.

Before your child can be considered for opportunities you NEED to complete and return to MovieExtras.ie three forms:

(a) Parental Consent Form 

Please complete a consent form which needs to be signed by a parent or legal guardian.

(b) School Principal Consent Form

If your child is going to school you will need to get a letter from the School's Principal.

If your principal has an objection to signing the consent form, ask them to send you an email outlining their concerns. Email this to Casting@MovieExtras.ie

PLEASE NOTE: You need to submit a new form every year in September. As the form is only valid for the current school year.

Returning the Forms

BY EMAIL – Scan or take a photograph of the form and email it to Casting@MovieExtras.ie

On Set Information

When you are booked for a production, you will need to bring on the day a completed On Set Information Form.

When you arrive on set, hand the form to either the extras co-ordinator / chaperone or other person who will be directing you for the day.
